PERSONAL – DECLARATION
I declare under my personal free and voluntary responsibility that I am member of S.P.P.SH (Union of State Police Employees), I do accept the Status and obligations and benefits derived from this Status and Collective Labour Contract.
In accordance with the request of point 2, of the law 117 of the Labour Code;
I do authorize the Employer to reduce from my monthly salary my membership fee of the sum of 300 (three hundred) lek, equal to 0.5% of the average salary of the police employees which is defined in the Statut to come to the bank account of the Union.
This authorization is permanent until the moment that, I, personally will ask in a written form from the Union where I am a member, the repeal of this declaration and the Union has the obligation that within 30 (thirty) days to make this declaration invalid.
This document is prepared in two equal copies where one of them is deposited to the Union and the other one to the police financial body where I work and I am paid.
